Appointments, Inpatient Admissions and Hospital Transfers
For appointments, inpatient admissions and hospital transfers, call M-LINE at 800-962-3555. For appointments only, you also have the option of filling in a Consult Request Form and faxing it directly to the clinic. For CRGs, visit the Consult Request Guidelines page on UofMHealth.org, or visit our PolicyStat website directly to search for CRGs related to a condition.

Provider Portal
The Michigan Medicine Provider Portal is a secure web-based application for referring providers and their staff to access their patient’s medical information, including clinic letters, discharge summaries, ED notes, test results, medical and problem lists, appointment notifications, etc. Visit the Provider Portal page on UofMHealth.org for information about how to enroll your practice. If you have already enrolled your practice in the Provider Portal, you can log in to the Provider Portal now.
